Adult Social Worker: Case Management (Flexible)

Caritas Recruitment is seeking a dedicated Adult’s Social Worker to join the Hackney Case Management Team. You will assess needs, develop tailored support plans and coordinate services to achieve the best outcomes for adults in the community.

The successful candidate will hold a social work qualification and be registered with Social Work England, with strong knowledge of adult social care legislation, excellent communication, and experience with case management tools.
