Frailty & Complex Care Social Prescribing Link Worker

Involve Kent is seeking a Complex Care and Frailty Social Prescribing Link Worker to join Medway Peninsula PCN and local GP practices. This 30-hour role is a fixed-term contract until 31 March 2027, with a salary up to £28,049 per year (pro rata based on hours and experience).

You will support people with frailty and complex health and social care needs, conduct CGAs, utilise KMCR, and connect clients to community services, with driving required for home visits.
