Northern Case Management is seeking an Associate Clinical Case Manager to join our team in the Bury area. The role focuses on networking cost‑effective services, assessing needs, planning and advocating for clients to improve quality of life.
You’ll work with paediatric and/or adult clients, develop rehabilitation and support plans, recruit and supervise support workers, and ensure continuity of care across home and community settings.
