Discharge and Flow Coordinator – Swindon

Overview

In this role, you help move patients safely home by coordinating discharge and flow within the ward and community services. You will work with nurses, therapists, social workers and a multidisciplinary team to ensure timely, well-communicated discharges and accurate records. The role focuses on supporting smooth discharge processes and building clear, compassionate communication with patients and families. You contribute to delivering safe, efficient care across our community hospital network and services, making a real difference to patients and carers.
