Dementia Adviser: Warm Community Support & Outreach (Lancashire)

Alzheimer’s Society in Lancashire is seeking a Dementia Adviser to provide warm, thoughtful support to people living with dementia and their carers. The role blends home-based work with regular travel across Central Lancashire, including local outreach and Singing for the Brain groups.

You will triage new referrals, manage your own caseload with careful records, and work with other professionals to ensure timely, person-centred help is in place.
