Extended Clinical Practitioner

Overview

In this role you will deliver extended primary care as an autonomous clinician within a GP group serving 49,000 patients. You will combine clinical assessment and decision-making with holistic care, referring to the wider MDT and leading extended primary care initiatives. You will run your own clinics, support policy delivery, and help shape new primary care models to meet patient needs. This position emphasizes leadership, collaboration, and innovation in a community setting.

Responsibilities

• Conduct initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
• Make safe clinical decisions and provide expert care within the practice
• Use holistic approaches for prevention and appropriate referrals to the multidisciplinary team
• Influence multi-disciplinary pathways to support patient choice, self-management, and early interventions
• Provide strong day-to-day leadership and creative leadership to enable a well-led team
• Develop and implement new concepts, models and practices to improve primary care services
• Collaborate with general practice and extended primary care teams to meet patient needs
• Support delivery of policy and procedures within the practice and community setting

Key requirements

• NMC or HCPC registration (professional UK registration)
• Degree level award
• Independent non-medical prescribing or pharmacology level 7 qualification
• Level 7 Clinical History Taking & Physical Examination
• Level 7 Diagnostics and Decision Making
• English language proficiency (written and verbal)
• Computer literacy for data entry and IT communications
