Cleckheaton- Same day acute clinics ANP/ACP

Overview

In this Locum role, you will deliver same-day acute care in a GP surgery setting, performing rapid clinical assessments and making differential diagnoses for undifferentiated presentations. You work within a supportive, collaborative practice to provide timely, safe care and appropriate treatment plans. You may complete up to two home visits, and you will prescribe as an Independent Prescriber to manage acute care pathways. This position offers flexible block bookings with a focus on high-impact urgent care delivery within primary care. You will contribute to a patient-centred, guideline-driven service that supports local urgent access networks.

Responsibilities

• Assess and manage same-day acute presentations, including face-to-face and home visits (up to two)
• Prescribe and manage acute care plans as an Independent Prescriber (V300)
• Triage urgent cases and coordinate referrals to secondary or community care
• Maintain real-time clinical notes in SystmOne
• Adhere to national guidelines, NHS safeguarding protocols, and professional codes of conduct

Key requirements

• Registered healthcare professional (NMC/HCPC)
• Current professional registration with the relevant governing body
• Master in Advanced Clinical Practice (or working towards it)
• Independent Prescriber (V300)
• Experience in GP or primary care settings
• Confident managing acute, undifferentiated presentations
• Comfortable working autonomously
