Lead Practitioner

Overview

In this role you lead specialist mental health work for children and young people, guiding care pathways within the THRIVE framework. You provide clinical oversight on shifts and support a multidisciplinary team across community bases serving Warrington, Halton, Knowsley & St Helens. The post supports development aligned with CYPMH strategy and national initiatives, with a focus on risk management, multi-agency collaboration, and high-quality care for families. Responsibilities

• Hold a caseload of complex assessments, treatments and consultations guided by a specialist framework and evidence-based practice
• Provide direct specialist therapeutic interventions including assessments, formulation and input for children, young people and families
• Conduct risk assessments and develop care plans within the THRIVE framework
• Authorise referrals to children and young people services, including urgent referrals
• Participate in duty rotas and provide guidance to colleagues
• Exercise autonomy within supervisory structures for assessment, treatment and discharge
• Deliver care coordination with effective assessment, planning and risk management
• Involve children, families and professionals through multi-agency meetings and networks
• Produce clinical formulations and intervention plans with child, young person and carers
• Maintain standards of clinical practice within specialist knowledge area
• Collaborate with team members to enhance service provision for young people and families
• Maintain knowledge of other modalities to engage with colleagues from different disciplines
• Provide consultation and guidance underpinned by specialist knowledge
• Promote effective multi-agency work and contribute to risk management in the network
• Undertake risk management for all children considering protective and risk factors
• Identify and respond to child protection issues per safeguarding guidelines
• Maintain accurate electronic clinical records compliant with DP Act and standards
• Engage with routine outcome measures and input data into clinical records
• Collaborate with children, young people and families and contribute to service user participation agenda

Key requirements

• Degree, or equivalent, or equivalent work-based experience
• Registration with relevant professional body; Appropriate Prescriber
• Evidence of CPD/short courses
• Experience in management and clinical leadership
• Experience of successful multi-agency working
• Ability to communicate complex information to patients, carers and colleagues
• Desirable: First line management experience
• IT literate
• Ability to work under pressure and manage diverse workload
• Excellent communication, organisational and interpersonal skills
• Risk assessment and report writing skills
• Ability to travel across boundaries
• Desirable: Research skills
• Understanding of multi-agency working
• UK professional registration currently holding
• Disclosure and Barring Service check
