Senior Practitioner

Overview

In this role you will provide clinical leadership within the Bolton North Specialist mental health team to deliver high-quality, recovery‑focused care. You will support the manager, guide staff development, and lead risk assessments and care planning for complex cases. The role requires strong communication and motivational skills to drive cross‑functional collaboration and service improvement. You will participate in service operations and contribute to the CPA‑guided care pathway to promote independence and well‑being.

Responsibilities

• Act as a keyworker for highly complex cases under the CPA framework
• Provide day‑to‑day clinical leadership to a multidisciplinary team
• Ensure delivery of safe, effective, high‑quality services across clinical pathways
• Contribute to Community Services Management Team for secondary mental health services
• Promote independence, social inclusion and recovery for adults living with mental health needs
• Participate in on‑call/on‑site rota as required

Key requirements

• Registration to professional body: OT, SW England NMC
• Extensive experience with adults of working age and complex needs
• Experience of MDT working and recovery‑focussed care
• Knowledge of CPA, Mental Health Act, Mental Capacity Act & Care Act
• Experience in assessments identifying health and social care needs
