Overview
In this Band 7 Clinical Lead role, you provide clinical and managerial leadership within the Neighbourhood Mental Health Team across Chichester and Bognor. You oversee complex cases, support timely discharges, and shape pathways between acute and community services. You collaborate with multidisciplinary teams and lead junior staff supervision to improve care nearer to home. Responsibilities
- Provide clinical and managerial leadership to the Neighbourhood Mental Team
- Oversee patients with complex needs and robustly manage discharge pathways
- Lead new admissions to acute hospitals and coordinate care from admission to post-discharge
- Attend and contribute to transition and discharge planning meetings (e.g., Crisis Team MDT, ward rounds)
- Liaise with carers, families, Acute Day Services, Havens, Staying Well service, and other organisations
- Collaborate with Lead Practitioners in hospital-facing teams to facilitate liaison with wards
- Supervise junior staff and provide professional support/supervision
- Support inpatient services to achieve timely discharges and appropriate care settings
- Work within the Trust’s People First ethos and promote care nearer to home
Key requirements
- Qualified and registered Social Worker, Occupational Therapist or Mental Health Nurse
- Experience in mental health settings
