Overview
In this role, you join the Mental Health Liaison Team in a general hospital to assess and manage mental health needs arising in the acute setting. You will work autonomously with patients experiencing acute or worsening mental health conditions, conducting holistic assessments and creating collaborative recovery-focused care plans. You will collaborate with a full multidisciplinary team to ensure timely risk assessment, care coordination and advocacy for patients. The position offers a rich learning environment with MDT discussions and opportunities to educate physical health colleagues, contributing to parity of esteem between mental and physical health.

Responsibilities
- Conduct comprehensive biopsychosocial mental health assessments and risk assessments with patients and carers
- Develop and implement ongoing care plans and management strategies within an acute hospital setting
- Provide specialist psychiatric liaison advice to acute hospital staff and coordinate resources
- Engage in multi-disciplinary team activities and contribute to round-table discussions and teaching sessions
- Ensure timely contact, documentation, and record-keeping with clear care plans and safeguarding awareness
- Promote confidentiality and adhere to professional and safeguarding policies
- Educate and support physical health colleagues about mental health care in the hospital setting
- Be accountable for own practice and participate in ongoing professional development and supervision
Key requirements
- Registration as Mental Health Nurse, Occupational Therapist, or Social Worker
- Experience in a mental health setting relevant to the post
- Effective communication with people who have difficulty understanding
- Good facilitation and interpersonal skills (active listening, empathy)
- Ability to organise self and others, manage workload and respond to behaviour that challenges
- Knowledge of Mental Health legislation and collaborative working with other agencies
- Willingness to engage in change management and recovery-focused, person-centred practice
