Hospital Social Care Practitioner: Discharge & Independence

Sheffield City Council seeks a Social Care Practitioner to join the Hospital First Contact Team at Northern General Hospital, working across Sheffield Teaching Hospitals sites. You will support patients from admission through discharge planning, applying strengths-based approaches and coordinating with healthcare and social care colleagues.

You will undertake assessments, provide information and options, and help families navigate care decisions to maximise independence and wellbeing.
