The post holder will be responsible for the development and delivery of a high quality service to ensure timely creation and maintenance of vascular access for patients with established renal failure, implementing and evaluating plans of care for a group patients in accordance with agreed policies and professional guidelines., The team covers clinics at both sites of SaTH inlcuding the Community Diagnostic Centre at Telford in Stafford Park. Our aim is for CKD patients to access the surgical pathway for the creation of vascular access.
The team works closely with Sonography and Radiology teams and liaise with vascular surgeons for vascular access plans.

The Renal Services is organized by a number of teams, all working together to provide a high quality services for our Kidney Disease population. This particular post works covering duties across the dialysis units at both sites: Shrewsbury Main Unit and Hollinswood House at Telford; including remote cover to Ludlow Dialysis Unit.
