Hospital Discharge & Integrated Care Team Manager

Herts Legal Service – Hertfordshire County Council in Hemel Hempstead is seeking a Post Hospital Review Team Manager to lead our Integrated Discharge Service, coordinating with NHS colleagues to support discharge pathways and promote independence.

You will provide strong operational leadership, manage performance, and develop a busy team of Social Workers, Occupational Therapists and Nurses within a seven‑day discharge service, with weekend rotation and a driving requirement.
