Empowering Homeless Health Support Advocate

Sirona care & health is seeking a Community Health Support Worker to join the Homeless Health team. The role supports people who are homeless or vulnerably housed, linking them to healthcare services within Bristol, North Somerset and South Gloucestershire.

You will work with registered clinicians to provide compassionate, person-centred care, helping clients access services and improve health outcomes while navigating complex health inequalities.
