Community Occupational Therapist — Home Rehab (Band 5)

NELFT NHS Foundation Trust is seeking a Band 5 Occupational Therapist to join a London-based community rehabilitation team. You will manage your own caseload of community patients, perform comprehensive assessments and devise person-centred treatment plans to help people regain independence at home.

You will collaborate with Physiotherapists, Nurses and Rehab Assistants, monitor progress, and adjust care to improve outcomes.
