Patient Care Navigator | GP Surgery Admin & Support

Cardiff and Vale University Health Board is seeking a Patient Health Care Navigator to join our GP surgery team. The role focuses on delivering a high-quality administrative service, handling inbound and outbound patient calls, and acting as the first point of contact for patients in a supportive and confidential environment.

You will support clinical and management colleagues to ensure the delivery of NHS Primary Medical Services while maintaining discretion and empathy in all interactions.
