Social Worker – Young Adults: Independence & Transitions

Lancashire County Council is recruiting two Social Workers to join the Young Adults Team within the LD&A Service of Adult Social Care in Lancashire. The role focuses on helping young people prepare for adulthood, coordinating support with partners, families and carers to promote independence and successful transition into adult services.
