Senior Community Nurse (Band 5) – Home Care & Coordination

Cardiff and Vale University Health Board seeks an experienced District/Community Nurse to deliver high-quality care to housebound patients in their own homes. You will work with the Community Nursing Team to plan and evaluate holistic care for patients, families and carers.

Flexible to support corporate approaches and maintain high patient standards. You will undertake first assessments, reviews and coordinate with other professionals, ensuring infection prevention and control.
