Home Treatment Team Practitioner

Overview

In this role you join the CAMHS Home Treatment Team to provide intensive, home-based support for 13–18 year-olds, helping prevent admissions and support safe discharges. You will work within a multidisciplinary CAMHS service that sits in the broader HPFT crisis pathway, contributing to high-quality, least-restrictive care. You will frequently collaborate with families and professionals to implement treatment plans and manage risk. This is a key part of the service expansion, offering a challenging yet impactful clinical role.

Responsibilities

• Assess clients appropriate for home treatment and develop/implement treatment plans within supervision
• Continuously review needs and seek supervision as needed
• Provide information, education, and support to families, carers, and professionals
• Maintain a client visits diary and complete required returns
• Develop expertise in acute care and risk management, including suicide risk
• Deliver profession-specific interventions tailored to service users
• Collaborate with MDT colleagues to maintain high standards of care
• Adhere to Trust policies, equality and anti-discrimination policies

Key requirements

• Qualified and registered Mental Health Nurse or Social Worker
• Diploma or Degree with extensive post-qualification experience
• ENB 998 or equivalent course
• Evidence of ongoing professional development
• Experience with young people and families in acute/crisis settings
• Experience with complex mental health needs, high risk, self-harm, and substance misuse
• Experience in a multidisciplinary team
• Ability to communicate sensitive information effectively
