Mental Health Practitioners: Referral & Assessment Hub

Greater Manchester Mental Health NHS Foundation Trust is seeking three Mental Health Practitioners to support the screening of referrals and assessments within the new Referral and Assessment Hub in Wigan. You will screen referrals for people with mental health difficulties and, with the MDT, determine the appropriate care pathway, including VCSE and Social Care.

These posts are full-time, 9am–5pm with a flexible rota; travel across Wigan and Leigh is required and some duty assessments may occur
