Senior Nurse, Hospital at Home – Flexible 21h Band 6

The Hospital at Home Senior Nurse – Band 6 is a 21-hour permanent post within the Division of Integrated Care, Adults, Children and Young People. You will work in patients’ homes as part of a multidisciplinary team to perform assessments, initiate treatments and coordinate care with colleagues.

You will practice autonomously within your competence, form management plans and support patients with acute, sub-acute and long-term conditions.
