Acute Frailty OT & PT Specialist — Holistic Care Leader

Calderdale and Huddersfield NHS Foundation Trust is seeking a Specialist Frailty Occupational Therapist/Physiotherapist to work within the Acute Frailty Service. The role focuses on comprehensive geriatric assessments and holistic care for frail patients in acute areas.

You will collaborate with an integrated multidisciplinary team across ED, MAU, SDEC and surgical divisions, delivering complex assessments and contributing to admission avoidance and reduced length of stay.
