Community Heart Failure Nurse Specialist (Band 5/6)

Hertfordshire Community NHS Trust seeks a Band 5/6 nurse to develop specialist knowledge in heart failure within the Integrated Heart Failure Service. You will work with Clinical Lead and Specialists to assess, plan and implement community nursing care for heart failure patients.

The role emphasises teamwork, strong communication and decision-making skills, with the service delivering five days a week community care and remote monitoring support where appropriate.
