Community Rehabilitation Worker – Home-Based Care (25 hrs/wk)

NHS Scotland in St Andrews is seeking an experienced Rehab Support Worker to join the Community Rehabilitation Team (CRT). You will deliver therapeutic programmes with the Occupational Therapy and Physiotherapy teams in patients’ homes, clinics and other community settings to prevent hospital admission and support early discharge.

You will hold SVQ Level III in Health and Social Care or equivalent; experience as a support worker and with older people is desirable.
