Home-Visit Community Support Worker

Wye Valley NHS Trust is seeking a Band 3 Community Support role to visit patients in their homes, supporting Band 5 Community Staff Nurses with observations, wound care and education. Must have access to a vehicle or be able to travel as required to meet service needs in Herefordshire and surrounding areas.

The role focuses on delivering high-quality home-based care, enabling patients to stay well at home and coordinating with primary and secondary care teams.
