Heart Failure Specialist Nurse – Community Care Lead

Sandwell and West Birmingham NHS Trust in West Bromwich is seeking a Band 6 nurse to join the Heart Failure Team. You will treat patients at home or clinic settings, coordinating holistic nursing care and supporting discharge planning.

Experience in heart failure care and patient/carer education is essential, with opportunity to specialise within the specialist heart failure team. A strong nursing background and clinical leadership are required.
