Clinical Lead, Neighbourhood Mental Health MDT

Pennine Care NHS Foundation Trust is seeking a post holder to work within the Neighbourhood Mental Health Team to assess and support the needs of people registered with a GP in the borough. You will lead a range of Pennine Care and VCSFE colleagues aligned with Integrated Neighbourhood teams and Primary Care Networks to ensure referrals are reviewed and signposted.

Daily team huddles and weekly MDT meetings will be supported.
