Stroke Specialist Nurse – Community Stroke Rehab Lead

East Coast Community Healthcare CIC in Great Yarmouth is seeking a Specialist Nurse to join the Integrated Community Stroke Service (ICSS). You will provide specialist nursing care, assessments, and coordinate multidisciplinary rehabilitation in the community.

The role includes medication reviews, basic mood screening, continence assessments and liaison with other services to support early discharge and improved outcomes.
