Social Worker – Adult Social Care Review Team

Central Bedfordshire Council is looking for a Qualified Social Worker to join their dedicated Adult Social Care Review Team.
You will work with adults with care and support needs, carers and significant others to understand what matters to them, identify their strengths and achieve positive outcomes through personalised care and support planning.
Key responsibilities:
Assess the needs of adults, families and carers
Develop personalised care and support plans
Review and evaluate interventions
Respond to safeguarding concerns and crisis situations
Work alongside NHS, Police, Housing, Education and voluntary sector partners
Apply relevant legislation, including the Care Act
Ensure practice is evidence-based and person-centred
Requirements:
Social Work qualification
Current Social Work England registration
Strong knowledge of adult social care legislation and best practice
Experience of assessments, reviews and safeguarding
Ability to work independently and as part of a multi-agency team
Working pattern: Monday–Friday, with flexibility depending on service requirements.
Rate: £40 per hour
