Social Worker: Intermediate Care & Home Support

North Yorkshire Council is seeking an experienced Social Worker to join the Intermediate Care team in Skipton. You will work with adults in their homes, supporting recovery and independence, and coordinating care with health and social care colleagues.

Role includes strength-based assessments, determining eligibility under national standards, and contributing to the quality of care while promoting wellbeing. AYSE support is available for newly qualified social workers.
