This post is a full time position (37.5 hours per week) and the working hours will be 9am – 5pm 5 days per week . The Hub is currently operational Monday to Friday 9am to 5pm so flexible working can be discussed. The post holder will be expected to travel across Salford and expected that base will be flexible. The post includes participation in the Bronze on call rota.
Key duties of the job
- Triage all referrals coming into the referral and assessment hub as part of a team. Providing an onward journey if not appropriate for Community Services.
- Completing assessments for those people requiring specialist mental health services under Community Mental Health Team or Neighbourhood Mental Health Team.
- To provide line management and Team management.
- To link in with CMHTs, NMHT and wider community services, and attend meetings as needed.
- To develop and maintain relationships and pathways with other services
- To promote awareness of the role of the referral and assessment hub, within local service area/team, GP practices and the wider community.

- Knowledge of requirements under the Care Programme Approach including the needs of carers and the mental health Minimum Data Set.
