About the role

We have an exciting opportunity for three full-time experienced and motivated Adult Support Co-Ordinators to join us as part of a dedicated 12-month programme supporting the implementation of a new Home Care Framework across Cambridgeshire.

This high-profile programme will play a key role in shaping the future of home care services for adults across the county, ensuring people receive high-quality, person-centred support that promotes independence, wellbeing, and positive outcomes.

As part of this programme, you will undertake reviews with adults currently receiving home care services across the following service areas:

Older People

Physical Disabilities

Learning Disabilities

Adult & Autism

This role provides a unique opportunity to contribute to a significant transformation programme, working directly with individuals, families, carers, providers and partner agencies to ensure support arrangements continue to meet people’s needs and aspirations.

You will be joining a dedicated team focused on delivering one of the most important programmes currently underway across adults.
