Mersey Care NHS Foundation Trust is seeking a Band 6 Mental Health Transition Practitioner to support young adults aged 18-25 transitioning into the CMHT/Recovery Team.
You will engage with the young adult, their carer and referrer, and conduct biopsychosocial assessments, acting as keyworker during the transition to the chosen pathway. The post supports NICE transition standards and team collaboration across services.
