Virtual Care Nurse — Independent Home Health & Telecare

Cornwall Partnership NHS Foundation Trust is seeking a Virtual Care/Virtual Ward Nurse/AHP to assess, plan, implement and evaluate care for patients at home, enabling independence and quality of life.
