SPOA Mental Health Navigator for Integrated Local Care

Dorset HealthCare in Dorset is recruiting an Integrated Care Practitioner to help connect Primary Care and Access Wellbeing Services within the Bournemouth and Christchurch Locality. The role focuses on screening, triage, and guiding referrals to appropriate interventions while supporting an integrated care pathway.

You will work across Primary Care Networks, supervise the SPOA model, and engage with virtual MDTs to progress patient care. DBS checks are required, with the Trust covering costs.
