Resettlement Lead Worker — Homelessness Support

Thames Reach Charity is seeking a Lead Worker for the Resettlement Support Team to assess clients, design move-on plans, and coordinate with partners across London to help people transition from street homelessness to stable housing.

You will document work, contribute to data collection, and build strong agency relationships while delivering a high standard of service. The role runs 37.5 hours weekly under a fixed-term contract until 31 July 2027.
