Overview
In this role you will deliver specialist clinical pharmacy support to community mental health teams in Torbay and Totnes, collaborating with primary care and mental health services. You will perform medication reviews, counselling, prescribing, audits, and training to enhance patient safety and outcomes. You’ll develop your mental health knowledge with structured induction and ongoing professional development in a supportive, values-driven Trust. Responsibilities
- Develop and provide specialist clinical pharmacy support to Mental Health community teams
- Build connections with local primary care (PCN) services and collaborate with other mental health services as needed
- Conduct medication reviews, counselling, prescribing, and audits
- Engage in physical health initiatives and training activities
- Support inpatient admissions for local patient groups where appropriate
Key requirements
- Up-to-date knowledge of guidelines and clinical practice in the specialty
- Experience working in a multidisciplinary healthcare team
- Ability to supervise pharmacy technicians and assistants
- Experience with change management and implementing change in complex settings
- Ability to work under pressure and meet deadlines
