Overview
In this role you will provide clinical leadership on medicines optimisation and safety within a general practice setting, backed by a senior pharmacist and GP supervision. You will work in a multidisciplinary team, handling chronic disease management and complex polypharmacy through clinical medication reviews. You will support prescription queries, medicines reconciliation, and safer prescribing, linking with community and hospital pharmacy to improve patient outcomes. This is a patient-facing role that contributes to care closer to home and integrated networks, offering meaningful impact in primary care within Haringey.
Responsibilities
- Carry out clinical medication reviews for older people with frailty and care home patients; make prescribing and monitoring recommendations to GPs.
- Manage medicines at discharge and reconcile post-discharge medications with patients and community pharmacists.
- Identify high-risk patients (e.g., polypharmacy, frequent admissions) and implement medicines optimisation strategies.
- Provide therapeutic advice to the practice team and signpost to community pharmacy for minor illness management and other services.
- Deliver training on therapeutics, medicines optimisation, and patient communication to the primary care team.
- Support clinical pharmacists’ networks and participate in faculty development for clinical pharmacy.
Key requirements
- MPharm and GPhC registration (essential).
- Experience in primary care and familiarity with GP systems (essential).
- Minimum 3 years post-qualification experience (desirable).
