Overview

As a Clinical Pharmacist within the Mewstone PCN, you will deliver medicines optimisation, clinical reviews, and safe prescribing across several GP practices. You’ll work in a multidisciplinary team to improve care quality and operational efficiency, with opportunities to pursue non-medical prescribing. You’ll contribute to public health initiatives and lead quality improvement through audits and service development. This role combines direct patient care with system-level prescribing support in a busy primary care network.

Responsibilities

  • Conduct clinical medication reviews and provide prescribing recommendations to GPs and allied health professionals
  • Manage medicines via transfer of care, repeat prescribing, and monitoring requirements
  • Lead medicines safety, audits, and quality improvement initiatives within the PCN
  • Deliver patient-facing clinics and telephone consultations about medicines
  • Support public health programs and development of new medicinal services
  • Collaborate with GPs, nurses, pharmacists, and wider team to ensure safe, effective prescribing
  • Analyse prescribing data and maintain formulary compliance across practices
  • Ensure regulatory and governance standards, including safeguarding and information governance, are met

Key requirements

  • Undergraduate degree in Pharmacy
  • GPhC registration
  • Understanding of professional practice standards
  • Desirable independent prescribing qualification or progression toward it
  • Experience in general practice or PCN setting preferred
  • DBS check and eligible for UK registration
