Overview

In this Pharmacist role, you will support hub-based primary care across multiple sites, contributing to chronic disease management and enhanced services. You will work closely with nurses and GPs, performing medication reviews and handling complex prescribing needs. The position offers mentorship and development within a forward-thinking, multidisciplinary team. You will help improve safety and outcomes through clinical queries, audits, and collaborative care. Responsibilities

  • Conduct chronic care reviews and assist with QOF targets
  • Perform virtual medication reviews for repeat prescriptions
  • Answer clinical medication queries from GPs, nurses and other professionals
  • Handle complex patient medication queries; liaise with secondary care as needed
  • Review hospital discharge letters and perform medication reconciliation
  • Manage complex medication queries from community pharmacies
  • Audit medication safety and suggest process improvements
  • Process shared-care requests from secondary care specialists
  • Prescribe/renew medications and switch acute requests to repeats
  • Review pathology results and conduct face-to-face medication reviews as indicated

Key requirements

  • Willingness or experience supervising junior pharmacists/technicians
  • Experience in chronic care conditions (Asthma, COPD, hypertension, T2DM)
  • Previous primary care experience
  • UK professional registration with GPhC
  • Independent Prescriber qualification
  • Desirable: minor illness experience; pathology management
  • Polite and confident
