Overview

In this PCN Clinical Pharmacist role, you will work across Holbrook and Shotley Practice and the wider PCN to optimise medicines, support clinicians, and be a key conduit for medicines information between hospitals and general practice. You will join a multidisciplinary team to assess patients, deliver medicines review services including domiciliary visits, and manage high-risk therapies. The position offers flexible hours and a chance to influence medicines management across a rural network. You will work closely with cross-functional teams to improve patient safety and care.

Responsibilities

  • Clinically assess and treat patients using expert knowledge of medicines across disease areas
  • Act as PCN point of contact for all medicine-related matters and build positive relationships
  • Standardise medicines management processes across the PCN where practicable
  • Consult and independently prescribe acute and repeat medication within defined competence
  • Manage referrals from triage services and clinicians
  • Resolve medicines queries from patients and staff
  • Provide medication reviews in-practice and during domiciliary visits to nursing homes
  • Manage caseload of complex patients
  • Oversee therapeutic drug monitoring and recall for high-risk drugs
  • Deliver long-term conditions clinics and home visits for patients with complicated regimens
  • Provide pharmaceutical consultations as part of the multidisciplinary team

Key requirements

  • GPhC registered Pharmacist
  • Hold or be working towards independent prescriber qualification
  • Hold or be working towards CPPE primary care pharmacy education pathway
  • Minimum of 2 years as a pharmacist
  • Experience managing pharmacy services in primary care
  • UK professional registration
  • Disclosure and Barring Service check required
