Overview
In this role you will join a General Practice MDT to provide expert medicines advice, independent prescribing, and medicines optimisation. You will lead on quality improvement, patient-facing reviews, and safe prescribing across multiple care settings, including care homes and domiciliary care. You’ll work closely with community and hospital pharmacy to improve outcomes and access to care, in a dynamic, patient-centred environment. This is a clinically senior role with scope for impact across QOF, public health, and service development.
Responsibilities
- Hold and run face-to-face structured medication reviews
- Prescribe independently and adjust medicines as needed
- Perform risk stratification to identify high-risk patients
- Run long-term condition clinics and care home reviews
- Manage medicines on discharge from hospital and continuity of supply
- Oversee repeat prescribing and monitoring
- Lead medicines safety and quality improvement projects
- Provide medicines information and training to staff
- Support public health campaigns and immunisation knowledge
- Ensure compliance with CQC standards and formulary guidance
- Collaborate with hospital and community pharmacists to optimise care
- Analyse medicines data to support decision making
Key requirements
- 3+ years post-graduate pharmacy experience
- GPhC registration
- MPharm and Independent Prescriber status or working towards it
- Immunisation status
- Excellent communication and interpersonal skills
- Good IT skills and ability to use databases and MS Office
