Overview
In this role you will provide autonomous, recovery‑focused mental health care within a collaborative multidisciplinary team. You will assess, plan and deliver evidence‑based psychosocial interventions to people with severe mental health difficulties in community settings, coordinating with families, carers and partners. You will support service development and act as a clinical leader to peers and students. A driving licence and vehicle are required for community travel. Responsibilities
- Conduct high‑quality assessments and develop personalised care plans
- Deliver evidence‑based psychosocial interventions to support recovery
- Manage risk and crisis planning with individuals and carers
- Build and maintain therapeutic relationships with clients and families
- Collaborate with GPs, inpatient services, social care and community partners
- Contribute to clinical leadership, supervision and team development
- Participate in service development, quality improvement, audits and projects
- Promote a positive, innovative team culture to deliver outstanding patient care
Key requirements
- RMN, Social Work or Occupational Therapy qualification
- Current registration with relevant professional body
- Significant post‑registration experience in multi‑disciplinary community mental health services
- Knowledge of resources and practices across health and social care
- Experience managing risk in crisis situations
- Experience with assessment, care planning, development and evaluation with clients and carers
- Experience and knowledge of working with people with mental health problems
- Strong communication and relationship‑building
