Overview

In this role, you will provide advanced clinical pharmacist services within a forward-thinking general practice. You will conduct face-to-face and remote consultations, lead medicines optimisation, and directly prescribe within your competence to improve patient outcomes. You’ll work closely with the Medicine Management team to deliver QOF and local prescribing targets, implement NICE guidance, and drive service improvements. This is a patient-centered, leadership-focused opportunity to shape medicines use across the practice and PCN.

Responsibilities

  • Undertake face-to-face and remote consultations
  • Perform structured and complex medication reviews
  • Independently prescribe medications within competence
  • Lead delivery of practice and medicines optimisation priorities
  • Support achievement of QOF targets and local contracts
  • Promote cost-effective prescribing and formulary compliance
  • Undertake audits and service improvement initiatives
  • Lead changes in evidence-based prescribing
  • Support training and development of the practice team
  • Manage repeat prescribing reauthorisation and post-discharge reconciliation

Key requirements

  • Degree in Pharmacy
  • GPhC registration
  • Independent Prescriber qualification
  • Experience in General Practice / Primary Care
  • Experience in clinical prescribing and medication reviews
  • Experience delivering medicines optimisation
  • Strong clinical knowledge of medicines and therapeutics
  • Excellent communication and IT skills (EMIS)
  • Ability to work independently and within MDT
