Social Care Officer

Overview

In this role you support adults in the community by assessing needs, planning care, and promoting independence. You will work within the neighbourhood team in Adult Social Care, liaising with customers, carers, and community agencies. The position emphasizes practical assessment, reviews, and enabling direct payments to empower service users. You join a supportive team focused on safeguarding and delivering results in a local government setting.

Responsibilities

• Carry out needs assessments and reviews to determine eligibility and support under the Care Act
• Promote independence and support through Direct Payments
• Engage with customers and carers in the community to co-create care plans
• Build and sustain links with community agencies to support service users
• Contribute to delivering outcomes and quality of care in a local government context
