Clinical Pharmacist

Overview

In this role you will apply clinical expertise and commercial awareness to support patients with chronic conditions in primary care. You will work with practices to deploy clinical tools, identify review opportunities, and ensure services align with our protocols, while increasing awareness of Interface Clinical Services within the NHS ecosystem. You will contribute to a patient-centred mission to improve long-term health outcomes and build lasting relationships with practices and local organisations. Responsibilities

• Support Practices with clinical tools and resources to manage patients with long-term conditions
• Ensure clinical services are delivered within protocols, operating instructions, and systems
• Identify opportunities for clinical reviews at practice, PCN, or ICB level
• Enhance awareness of Interface Clinical Services within primary care and develop relationships with practices and local NHS organisations

Key requirements

• GPhC registered pharmacist with a minimum of 2yrs PQE
• Primary care experience desirable
• PCPEP qualification desirable
• Full UK driving licence
• Willing to travel as required, with remote (home) working
• Keen passion to make a positive difference to patients’ long-term health
• Driven work ethic
