Overview
In this Clinical Pharmacist role, you will improve patient health outcomes by delivering medicines expertise within a multi-disciplinary primary care team. You will proactively address medicines optimisation, drive safe prescribing, and support complex polypharmacy management. The post sits at the heart of care integration across PCNs, with a focus on reducing health inequalities and enhancing outcomes. This is an opportunity to shape medication use in primary care while collaborating with pharmacists, GP teams, and community partners.

Responsibilities
- Meet with patients and assess treatment needs in a multidisciplinary setting
- Pursue prescriber training and practice under supervision within the GP team
- Manage chronic disease care and conduct structured medication reviews to optimise therapy
- Provide medicines expertise to address public health and social care needs
- Lead medicines optimisation and contribute to quality improvement and antibiotic stewardship
- Promote adherence, reduce waste, and support self-care through medication reviews
- Support integration of general practice with wider healthcare teams
- Build relationships with pharmacy professionals across PCNs and health and social care systems
- Lead/participate in shared care protocols, clinical research, and liaison with specialists
- Engage in professional clinical networks with monthly/quarterly supervision arrangements
- Ensure appropriate supervision structures and peer support within the team
- Undertake clinical audits as agreed with the Prescribing Lead
Key requirements
- Experience of working in primary care
- Experience of working in a GP practice
- Strong organisational and IT skills
- Registered with GPhC
- Desirable: Independent Prescribing, CPPE Primary Care Pathway, EMIS, Docman, Accurx
