Overview
In this role you will split your week between acting as the responsible pharmacist in our community pharmacies and delivering frailty services within CHCP. You will work with a multidisciplinary team to deliver comprehensive geriatric care across community settings and hospital-at-home initiatives. You will contribute to NHS pharmacy services, including vaccinations and medicines management, while developing in a supportive, values-driven environment. This is a hands-on, patient-centered opportunity to shape elderly care delivery at scale.
Responsibilities
- Act as responsible pharmacist in community pharmacies
- Deliver frailty services as part of CHCP frailty team
- Collaborate with multidisciplinary team across community homes, care centres, and hospital-at-home settings
- Support delivery of NHS pharmacy services (Pharmacy First, vaccinations, NMS, DMS)
- Engage with patients, GPs and healthcare professionals to ensure high-quality care
- Participate in induction and ongoing professional development
- Travel across service areas and work flexibly to meet organisational needs
Key requirements
- MPharm (or equivalent pre-2000) and GPhC registration
- Evidence of Continuing Professional Development
- Ability to apply clinical knowledge across diverse patient groups
- Understanding of medicines policies and legislation
- Strong analytical and decision-making skills; proficient with MS Office
- Experience in influencing, working on projects with initiative, and multi-disciplinary teamwork
