Hertfordshire Community NHS Trust is seeking a band 5 or band 6 nurse to support heart failure patients within the Integrated Heart Failure Service (IHFS) in the community. You will work with Clinical Lead and Specialists to assess, plan and deliver specialist nursing care, focusing on medication titration, admission prevention and discharge support.
The role is team-based, with emphasis on communication, decision-making, and developing specialist knowledge in heart failure.
